As an old man leaves the 'consulting room', he slips a coin into the waiting hand of a young woman in a white dress and feathers. He gives her a leery look while she casts an eye over her shoulder and unlocks the door, all aiding an impression of subterfuge.

Outside a Jewish man can be seen explaining his complaint to the 'physician's' servant girl. The door carries the legend Old Sarum, the most notorious of the rotten boroughs prior to the Reform Act of 1832 from which its owner was able to derive a considerable income. A paper in the old man's pocket reads 'Writ of Ejectment Retaining Fee'.
