Toilet implement
This toothpick would have been part of a toilet or cosmetic set. It has a hole at the end of the handle to attach it to the rest of a set of cosmetic tools. Although medieval doctors were uncertain whether bathing was good for you ('in time of pestilence avoid baths'), objects such as toothpicks show that medieval Londoners cared about personal grooming.
