The Hearts of Oak Benefit Society was registered under the Friendly Societies Act which, before the establishment of a National Insurance scheme, was approved by the government to administer mortgages and national insurance benefits.
Agents of the Hearts of Oak Benefit Society used this form or 'Sick List' to record a member's sickness, the types of medical certificates obtained and other details of an agent's visit. This information would be used to calculate the amount of benefits to be paid to the member. The form is unused, but has an ink stamp for an East Ham agent.
